Liability Waiver & Informed Consent - General
Paws & Pawsibility LLC will endeavor to create as safe an environment as possible for my dog's training and will offer only sound, safe, and responsible training and training instructions. However, to the extent that Paws & Pawsibility LLC is insured for any unintentional or negligent errors, omissions, or incorrect assertions, will be responsible for any such acts or omissions, but only to the extent of such insurance. I have been told by Paws & Pawsibility LLC and understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself or others, and consequently I am and will remain responsible for the actions of my dog at all times. I hereby agree to indemnify and hold harmless Paws & Pawsibility LLC of any and all claims of injury, expense, costs, or damages caused by my dog.
I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service.
Additional Liability Waiver & Informed Consent - Group Classes
I hereby agree to abide by the rules and policies of Paws & Pawsibility LLC group training classes as outlined in the contract. I understand that attendance of dog training classes is not without risk to myself, members of my family, guests who may attend, or to my dog. In consideration of, and as an inducement to the acceptance of my application for training membership in this training class, I hereby agree to indemnify and hold harmless Paws & Pawsibility LLC, its officers, directors, instructors, agents, employees and/or representatives of any and all claims, or claims by any member of my family, or accompanying guests of mine of injury, expense, costs or damages to myself, my dog or any handler sponsored by me both in class and out of class. In addition, I agree that I will defend and indemnify Paws & Pawsibility LLC for any injury, expense, costs, or damages to any dog handlers or dogs, whether sponsored by me or not, or to third parties arising out of my own actions or the actions of my dog. I have read the above-stated provisions, and agree to accept those responsibilities.
Payment Policy
I understand that payment for services are due paid in full at the time of booking prior to any services performed.
Payment can be made online with a debit or credit card at the time of registration.
Cancellation Policy - Consults/Private Coaching Sessions/Packages
Paws & Pawsibility LLC understands that circumstances may change suddenly or unexpectedly. For a full refund of Private Coaching Session/Package fees and tuition paid in advance, I understand that cancellation notice must be received by Paws & Pawsibility LLC by phone or email at least 5 days (120 hours) in advance of the first scheduled coaching session.
I understand that once the Private Coaching Package has started, tuition is nonrefundable. I am responsible for payment in full regardless of whether I choose to complete the training program.
I understand that if I need to cancel or reschedule a Consult or Private Coaching Session, notice must be received by Paws & Pawsibility LLC by phone or email at least 24 hours in advance. Late cancellations, made less than 24 hours in advance, will be charged in full for the canceled session.
Private Coaching Package purchases expire 3 months (90 days) from the date of purchase.
Cancellation Policy - Group Classes
Paws & Pawsibility LLC understands that circumstances may change suddenly or unexpectedly. For a full refund of Group Classes fees and tuition, I understand that a cancellation notice must be submitted by phone or email at least 24 hours in advance of the 1st class. Fees and tuition are nonrefundable less than 24 hours before the 1st class or once classes have commenced.
If I must fully withdraw from all remaining classes after the series has commenced due to unexpected human or dog injury or illness, I understand that Paws & Pawsibility LLC will issue class credit pro-rated for any unattended classes in the series. These credits can be applied to Group Classes or Private Coaching Sessions. Credits must be used within 180 days (6 months) or they are forfeit.
Group Class Make-Up Policy
I understand that in order to get the full benefits of class, my regular attendance is crucial. Paws & Pawsibility LLC is unable to offer make up classes, however, I may have another person attend class in my place. Homework lessons will be emailed for the class missed.
Dog Behavior - Group Classes
I understand that not all dogs are suitable for group classes. If my dog shows aggression towards people or other dogs, or is disruptive to the class, they may be dismissed from class without a refund.
Owner Commitment
I understand that Paws & Pawsibility LLC is committed to helping dogs and their owners through the teaching and employment of science-based training techniques and principles. These techniques require commitment and consistency from all members of the family for the dog to be successful. Training is a commitment for the entire course of a dog’s life. As the owner, I understand that my involvement and commitment to this process are critical to the success of any training sessions that occur. Paws & Pawsibility LLC reserves the right to terminate this contract if the owner(s) are not cooperating with and/or implementing the suggested advice and practice.
Private Coaching Packages
I understand that Paws & Pawsibility LLC Private Coaching Packages expire 90 days (3 months) from purchase. This includes any rescheduled training sessions. Any sessions from a Private Coaching Package that are not used within 90 days of purchase are forfeited.
COVID-19 Policy
I warrant and represent that I will remain aware of and abide by local law and public guidance on reducing COVID-19 exposure and will follow best practices regarding Paws & Pawsibility LLC’s services regarding social distancing and sanitation.
